The excision of any lesion, benign or malignant has the inevitable sequelae of scarring. Epidermal inclusion cysts such as sebaceous cysts or epidermal cysts, and milia are frequently occurring lesions that are most commonly excised surgically. Epidermal cysts are benign lesions of the skin and reasons for removing them can be functional (depending on site and size), cosmetic or due to multiple infections. Surgical excision is the conventional method of removal, however, when it comes to large epidermal cysts in aesthetically sensitive areas such as the face and neck, an aesthetically pleasing result can often be difficult to achieve as it may lead to a poor scar or poor contouring.

Introduction: Traditionally, either a wide excision is performed or a more limited incision with the goal to remove the cyst in its entirety, including both anterior and posterior walls. The aim is to avoid the possibility of recurrence and infection. The former technique will lead to a potentially long scar, but ultimately, both can lead to poor scarring and potential poor contouring. There are a number of surgical techniques described in the literature [1], all which aim to minimise the length and visibility of the scar. These techniques were characterised as minimal excision techniques. Wu H et.al were the first to mention CO2 laser in the treatment of sebaceous cysts, but this was for access rather than definitive treatment [2].

We therefore put forward this novel way of treating sebaceous cysts and milia, by use of Carbon Dioxide laser as a definitive treatment by ablation, which has not been described. We present a case series of four patients, all of which were treated with CO2 laser in one setting and present the resultant promising cosmetic outcomes. Though neither the pathology of the cysts nor the modality of the CO2 laser are unique, the treatment approach.

The two week wait pathway initiative was proposed in 1997 to expedite referrals from general practice to dermatology and to facilitate access to timely treatment for patients with suspected skin malignancy. However, since its implementation it has come under fire for its poor specificity and case detection rate with dermatologists raising concerns about their increased workload and consequently lack of time for other urgent cases. The present study conducted at Queen’s Hospital, London aimed to assess (i) the proportion of referred patients seen within two weeks and treated within 62 days (ii) the proportion of patients referred for suspected malignancies that were subsequently confirmed and (iii) what benign skin lesions were most commonly referred to dermatology. We audited 83 patients who were referred to Queen’s Hospital between April to June 2019 under the twoweek wait pathway. Data was collected using patient notes, EPro, Cyberlab, and BlueSpier.

The results show that two-week wait referrals for suspected skin cancer from GP's have a low diagnostic accuracy and this may be contributing to delayed patient care within the hospital setting. In previous studies conducted in the United Kingdom, approximately 20% of GP referrals for squamous cell carcinoma or melanoma using the two-week wait proforma are later diagnosed through histopathology. In this study, 13.2% of patients referred were found to have squamous cell carcinoma, basal cell carcinoma or melanoma. The three main diagnostic groups in this study were: seborrhoeic keratosis (26.5%) benign naevus (20.5%) and basal cell carcinoma (9.6%). Furthermore, only 65% of patients referred had a dermatology appointment within 14 days of referral and 75% of patients requiring treatment were treated or scheduled to be treated within 62 days. These results suggest that greater knowledge and confidence is required amongst GP's in the Queen's Hospital catchment area to improve diagnostic accuracy in patients with skin cancer. It is likely that a combination of education and resources will be required to meet the demands of this population effectively.

Angioedema (AE) is an uncommon condition which can be life-threatening in case of laryngeal involvement or during anaphylactic shock. The studies concerning Ae are rare in sub-Saharan Africa. Ours aims were to determine the epidemiological-clinical characteristics and etiological factors of AE and to describe the management modalities in Dakar.

We conducted a descriptive and analytical study range from March 1st to August 31st 2017 in two dermatology departments (Institut d’Hygiene Sociale, Aristide Le Dantec Hospital) and one cardiology department (Aristide Le Dantec Hospital).

We included all patients presented with AE and who consented to participate in the study. During the study period, 37 patients were included, which standing for a 0.28% hospital Frequency. The mean age of our patients was 38.51 years old woth a sex ratio of 0.6 . The idiopatic histaminergic Ae was the predominant type with a frequency of 86.48% (n = 32). The allergic cause was present in 32.43% (n= 12). We have incriminated pseudo-allergy in 18.92% (n = 7). We found 5 cases of AE associated with medication in 13.51% of cases. The Angiotensin-converting enzyme (ACE) inhibitors were found in 4 cases and ARAII antagonist in 1 case The Angiotensin-converting enxyme (ACE) inhibitors were found in 4 cases and ARAII antagonist in 1 case . Different factors were identified food in 35% (n=3), medicinies in 29.73% (n=11) and physical exericise in 5.4% (n=2). The lips (43.24%, n = 16) were the predominant localization. Urticaria was present in 37.84% of cases (n=14), all of Whom had histaminergic AE. We found a statisrically significant relationship between urticaria and histaminergic AE (p = 0.03). Prick tests were positive in 13 cases with clinical concordance in 8 cases. The determination of IgE specific trophallergens allowed us to highlight a food allergy in 2 patients.

For histaminergic AE cases, treatment was based on anti-H1 exclusively in 29.73% (n = 11) or associated with oral corticosteroids in 35.13% (n = 13). For cases of bradykinic mediated AE, there was a change in medications along with a clinical follow up

Two monstrous female health-related issues are currently engaging the attention of the global community viz, (i) the chemical skin-lightening issue and (ii) the rising female breast cancer issue. Since the skin-lightening chemicals are notable endocrine disruptors, a research question has been entertained for a possible link between skin lightening cosmetics and breast health.
Mercury and hydroquinone have been used as skin lightening agents for more than five decades since the inception of the skin bleaching practice. Methylmercury is IARC Group 2B carcinogen. Methylmercury and hydroquinone have immunotoxic, embryotoxic and endocrine disruptor (ED) effects, hence, breast cancer risk factors. Mercury chloride has shown some carcinogenic activity. Hydroquinone causes the release of Trimethylamine (TMA) and N-Nitrosodimethylamine (NDMA), members of the family of extremely potent carcinogens which are EDs hence risk factors for breast cancer. Skin bleachers release fumes of TMA and NDMA which may be of environmental public health importance (like tobacco smoke) to non-bleachers. Oxybenzone and Titanium dioxide, the main ingredients in sunscreen chemicals, often incorporated in skin-lightening products are carcinogens and Endocrine disrupting chemicals, hence breast cancer risk factors. Other notable EDs in skin lightening cosmetics are phthalates, and heavy metals contaminants. These chemicals are modifiable breast cancer risk factors; there is need for public health approach to long-term prevention of breast cancer.

Conclusion: Mycetoma cutis is less common in republic of Yemen and this is the first case reported.
